The Norfolk Primary Care (NPC) Norwich Home Visiting Service provides high-quality care to patients aged 18 and over who are registered with one of the following GP practices in Norwich:

How It Works

If you are unwell and unable to attend your GP surgery, your practice may refer you to our Home Visiting Service. A skilled clinician from our team will visit your home to:

Assess your condition
Provide treatment
Make referrals to specialist services if necessary

We work closely with your GP practice to ensure you receive the right care, at the right time, in the comfort of your home.

Service Hours

Home visits are available Monday to Friday from 10:00 AM to 6:00 PM, excluding bank holidays.

What To Expect During Your Visit

Flexible Scheduling: Visits are planned between 10:00 AM and 6:00 PM, but exact times are not fixed to accommodate urgent needs.
Professional Care: A clinician will arrive at your home, introduce themselves, and carry out a full assessment using the necessary equipment.
Collaborative Planning: Your care plan will be created in partnership with you and your GP practice and recorded in your medical record.
Comprehensive Documentation: The visiting clinician will update your GP practice with details of your consultation to ensure seamless continuity of care.

Who Provides the Service

Norfolk Primary Care (NPC) is a Community Interest Company dedicated to supporting GP practices across Norfolk and Waveney. Our goal is to ensure you receive the right care, at the right time, in your preferred setting.

The service is operated by:

Service Lead
Visits Coordinator
Clinicians: Each day, a team of five clinicians, including GPs, Advanced Nurse Practitioners, and paramedics, deliver home visits.

NPC works collaboratively with GP practices and support staff to provide excellent primary care services tailored to your needs.
