Improved Access and Extended Hours

The ECHT was commissioned in 2018 to support the 19 GP practices with their Care Homes and housebound patients to offer primary holistic care, avoiding unnecessary hospital admissions and supporting those vulnerable patients to stay in their preferred place of care.

The Team was nominated and won the Eastern Region Parliamentary award for Excellence in Primary care 2021.

Who is it run by?

Norfolk Primary Care oversees the administration of this service through the following people:

Natalie Catley – Service Lead
Libby Catchpole – Project Manager
Amy Pemberton – Clinical Services Support

The service is run by our team of Advanced Nurse Practitioners who work closely with the patient’s GP Practice

Where is it run from?

The Enhanced Care Homes Team is based in their designated GP practice for the day carrying out visits on behalf of the GP Practice.

When is the clinic run?

The service is run, Monday to Friday 09:00-17:00, excluding Bank Holiday’s.

How do I get an appointment?

The care homes will arrange all appointments with the patient’s registered GP Practice, however, if you are housebound and wish to have an appointment this can be done by contacting your practice directly.

How do I contact the service?

Please contact your surgery directly for any queries related to the service. These can either be dealt with there directly or passed on to Norfolk Primary Care for a response.

What do patients think about our service?

Our patients are grateful to have a clinician who has the time to assess, treat and make onward referrals as necessary.
